I confirm that my child’s
health meets the physical standards for participating in the activities
involved in the Tennis Camp. I understand that neither the Anguilla Tennis Academy
or the coaches and staff of the camp will assume responsibility for accidents
resulting from training during my child’s participation. I give my
consent and approval for the Anguilla
Tennis Academy,
its Director, and staff to act on my behalf in securing medical attention for
the above applicant from a licensed physician or hospital.
